# An unknown sample. Peptide Laboratory Evidence Review

> Peptide Laboratory Evidence Review starts with a seized sample and follows CJC-1295, tesamorelin and MOTS-c into blood tests, body scans and unanswered claims.

A seized preparation acquired a chemical identity. I like an instrument with a clearly stated job.

## Chemical identification

A preparation seized in an anti-doping case had an uncertain identity. In a 2010 paper, [high-resolution LC-MS/MS identified CJC-1295 in the material](https://doi.org/10.1002/dta.233). The initials describe an analytical method, liquid chromatography with tandem mass spectrometry. The result was a name for the substance in that sample.

Such a modest analytical success appeals to me. It set out to identify something and did so. The satisfaction is real, even though a chemical identity leaves purity, safety and usefulness as separate questions. A machine can be very convincing within its remit.

The more interesting trouble starts when that confidence travels. An identified peptide becomes a story about recovery; a higher hormone value becomes a story about feeling younger. The measurements in between give me much more to puzzle over. [CJC-1295 blood studies](https://pubmed.ncbi.nlm.nih.gov/16352683/), [tesamorelin scans](https://pubmed.ncbi.nlm.nih.gov/25038357/) and [MOTS-c measurements in people receiving dialysis](https://pubmed.ncbi.nlm.nih.gov/39111290/) offer quite different answers. That variety is more absorbing than a catalogue of promised effects.

## A hormone followed over days

CJC-1295 has an unusually persistent form. An attachment called DAC allows it to bind albumin, a plentiful blood protein. In [healthy adults given the long-acting form](https://pubmed.ncbi.nlm.nih.gov/16352683/), average growth hormone rose 2- to 10-fold for 6 days or more, and IGF-1 rose 1.5- to 3-fold for 9–11 days.

Then there is the rhythm. A separate study found that [growth-hormone pulses persisted while the level between pulses rose](https://pubmed.ncbi.nlm.nih.gov/17018654/). That raised baseline surprised me. A smooth plateau had seemed more plausible. The [CJC-1295 page](/cjc-1295) follows that raised baseline, the different forms and the safety questions that a lasting hormone response creates.

## Abdominal measurements

Tesamorelin makes this a story about anatomy. A trial in adults living with HIV and abdominal fat accumulation [measured changes in visceral fat and liver fat](https://pubmed.ncbi.nlm.nih.gov/25038357/). Visceral fat is the fat around internal organs, an area a scan can distinguish from the fat beneath the skin.

For me, location makes the result worth examining. The trial can say where fat changed and in whom. Tesamorelin has [a specific approved use for excess abdominal fat in HIV-associated lipodystrophy](https://www.ncbi.nlm.nih.gov/books/NBK548730/); the findings do not establish general weight loss. The [tesamorelin page](/tesamorelin) stays with the scans, their units and the return of visceral fat after treatment stopped.

## Dialysis blood

MOTS-c brings a different complication. It is a mitochondrial peptide, studied in energy use and cellular stress. In a cohort of people receiving long-term dialysis, [higher circulating MOTS-c was associated with mortality and cardiovascular risk](https://pubmed.ncbi.nlm.nih.gov/39111290/). The participants were observed; nobody was given MOTS-c as an intervention.

That result resists an easy “more is better” story. I have no urge to discard that association merely because [MOTS-c improved physical performance in mice](https://pubmed.ncbi.nlm.nih.gov/33473109/). The human blood finding and the mouse treatment finding describe different situations. [MOTS-c](/mots-c) deserves room for both.

## Investigations

- [How long did the hormone response last? CJC-1295](/cjc-1295)
- [Where did fat change? Tesamorelin](/tesamorelin)
- [Why was a higher blood level linked with risk? MOTS-c](/mots-c)
- [Has the actual combination been tested? CJC-1295 / Ipamorelin](/cjc1295-ipamorelin)

The [comparison desk](/compare) puts those questions alongside their measurements. The [reading list](/references) includes the analytical paper, the human trials and the cell and animal work.
